Veterans Inc. – Agency Referral Process (Quick Guide)
Two Steps Required
The Admissions team cannot begin review until both steps are completed.
STEP 1: Submit Referral in AdvancedMD
- Select “I am NOT the patient”
- Enter your contact information, including under the email (all status updates willbe sent here)
- Enter patient details:
- Name, DOB, SSN, phone number
- Insurance
- Upload documents (ID, insurance cards, medical records)
- If unable to upload → Fax: 855-710-7852
- Accept consent, select program/location, add discharge date under additionalnotes, click Submit
STEP 2: Complete Agency Referral Form
- Check email (within 1–2 minutes) → open “Confirmation of Referral Submission –Additional Information Needed”
- Click link, complete Microsoft Form, click Submit
AFTER SUBMISSION
All communication goes to the email entered in AdvancedMD. You will receive:
- “Referral Review Completed” email
- Final decision email (acceptance with next steps OR denial with reason)
IMPORTANT
- Both steps are required – incomplete referrals delay review
- Missing clinical documents delay medical review
- Submit one referral per patient
- Do not resubmit for updates – contact Admissions
SUMMARY
- Submit referral request in AdvancedMD
- Complete Microsoft Form
- Monitor email for decision
